Condition
Sesamoiditis
Inflammation of the small bones under the big toe joint causing forefoot pain.
What It Is
Understanding Sesamoiditis
On the bottom of the Metatarsal Phalangeal joint or big toe joint are two pebble sized bones called Sesamoid bones. These are important bones that help guide proper big toe and overall foot biomechanics and help to stabilize and disperse forces during weightbearing activity. When the forces in and around the Sesamoids are excessive the soft tissues surrounding these little bones can become inflamed and painful. This is referred to as Sesamoiditis. We use Softwave – Extracorporeal Shockwave Therapy to reduce toe pain and inflammation at the cellular level and also to heal degenerative and weakened soft tissues. Equally important is identifying the underlying cause of the excessive loading under the big toe because unless the root cause is corrected it will continue to wreak havoc on the big toe joint and many other structures in the foot and ankle complex.
Symptoms
What You May Be Experiencing
- Pain directly under the big toe joint
- Pain that worsens with push-off, running, or jumping
- Swelling and tenderness in the ball of the foot near the big toe
- Difficulty bending the big toe upward
- Bruising or aching feeling deep under the joint
The Problem
Why Traditional Treatments Often Fall Short
Rest, padding, and stiff-soled shoes can help reduce loading, but the area never gets enough opportunity to fully heal because we walk every day. Cortisone injections near the sesamoids carry real risk to surrounding tendon tissue, and surgical removal of a sesamoid is a serious procedure with long recovery.
Our Approach
How SoftWave® Helps Sesamoiditis
SoftWave® shockwave therapy stimulates healing of the inflamed soft tissue and bone interface around the sesamoids, improves circulation to a notoriously slow-healing area, and breaks the cycle of chronic irritation. Patients are commonly able to return to running, dancing, and full activity without invasive intervention.
FAQ
Sesamoiditis — Common Questions
Do I have to stop running during treatment?
We modify activity during the treatment series and progressively reintroduce loading as the tissue heals.
What if I have a stress fracture in the sesamoid?
We’ll review imaging and adjust the protocol — SoftWave still has a role, but acute fractures need careful management.
How long until I'm back to normal?
Most patients feel meaningful change within 4–6 sessions, with full return to activity over the following weeks.
