Posterior Tibialis Tendon Dysfunction

Failing arch-supporting tendon causing inner-ankle pain and flatfoot collapse.

What It Is

Understanding Posterior Tibialis Tendon Dysfunction

Pain on the inside of the lower leg, ankle, and/or foot, especially with walking, may be another painful foot condition that needs treatment as soon as possible. The Posterior Tibialis Tendon plays some critical roles in our ability to stand and walk amongst most other weightbearing functional activities. Foot pain can be an early warning sign that something potentially life altering is brewing. Posterior Tibialis Tendon Dysfunction can progress to a condition called Adult Acquired Flat Foot Deformity or more recently known as Progressive Collapsing Foot Deformity which is characterized by collapsing arches and permanent damage to other important structures that hold our medial foot and ankle together. The most critical initial treatments include 2 things; 1.) Softwave – Shockwave Therapy to begin healing and improving the structural integrity of the Posterior Tibialis Tendon and 2.) Protection of the tendon with VERY SPECIFIC footwear. As with many other conditions that cause foot pain, early Posterior Tibialis Tendon treatment is important and necessary to prevent progression to later stage functional disability. Once renewed healing is underway the strategy begins to incorporate strength and arch stabilization exercises to make the Posterior Tibialis Tendon and all of the other muscles of the foot and ankle complex strong and robust and resilient.

The Problem

Why Traditional Treatments Often Fall Short

Bracing and orthotics support the foot externally but don’t restore tendon health. Cortisone is generally avoided in this tendon because of rupture risk. Reconstructive surgery is invasive, with long recovery and significant lifestyle change — and is best reserved as a last resort.

Our Approach

How SoftWave® Helps Posterior Tibialis Tendon Dysfunction

SoftWave® shockwave therapy stimulates collagen remodeling within the posterior tibialis tendon and surrounding tissues, restoring strength and resilience that bracing and rest cannot produce. Combined with progressive loading, patients commonly regain arch support and avoid reconstructive surgery.

FAQ

Posterior Tibialis Tendon Dysfunction — Common Questions

If caught before advanced collapse, yes — function and shape can improve significantly. Advanced cases focus more on pain and function than full arch restoration.
Yes, especially during treatment. We progressively reduce dependence as the tendon strengthens.
Yes — partial tears are exactly what SoftWave is designed for. Full ruptures need surgical evaluation.
